I, the undersigned, being the parent or legal guardian of the minor named below, do hereby consent and authorize Montani Studios ("Photographer"), its legal representatives, assigns, and those acting with its permission, to use and publish photographs of said minor, or in which said minor may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium.

I warrant that I am the parent or legal guardian of the minor and have the authority to execute this release. I hereby release and agree to hold harmless the Photographer from any and all liability arising out of the use of the photographs.
